Registration
Please read this page carefully before completing the registration form.
How to register
The quickest and easiest way to register is online. We encourage all delegates to register online for instant confirmation.
Alternatively, fill in the registration form and return to the Secretariat. Each delegate must complete a separate form. Please read this registration brochure carefully before completing the registration form. Forms will be accepted by post, e-mail or fax.
Registration fees
All fees are quoted in Australian dollars (AU$) and are inclusive of 10% GST. Registrations made after 24 October 2012 must be made onsite at the Congress.

* Members of iADH, ASSCID and ANZSND are entitled to the member rate. You must be a current registered member of these organisations to be eligible for the member rate.
** Please note that a copy of the delegate’s student ID must be faxed or e-mailed to the Congress Secretariat within one week of registration or the rate will be moved to full delegate.

Full registration (iADH Members, non-members and Oral Health Practitioners) includes:
- Attendance at all sessions
- All catering in the Exhibition
- Name badge
- Satchel including program

Post-graduate, Student and Developing Nations registration includes:
World Bank List of Developing Nations
- Attendance at all sessions
- All catering in the Exhibition
- Name badge
- Satchel including program
- Welcome reception (Sunday)
Day Registration - delegates includes:
- Attendance at sessions on that day only
- Catering in the exhibition on that day only
- Name badge
- Satchel including program
Accompanying Partner Registration includes:
- Name badge
- Accompanying persons bag
- Accompanying persons tour
- Welcome reception (Sunday)
- Gala Dinner (Tuesday)
Method of Payment
Payment of fees must accompany all registrations. No registration will be confirmed until payment is received. Payment can be made by the following methods:
Credit cards: Visa, MasterCard, American Express and Diners Club
Personal or company cheque (Australian delegates only) and made payable to iADH2012 and must be received prior to 18 October 2012.
Bank drafts and international money orders should be in Australian dollars, drawn on an Australian bank and made payable to iADH2012. Please fax a copy of the bank remittance to +61 2 9251 3552.
Payment Deadlines
Registrations received by the 23 January 2012 must be paid in full by 23 January 2012 in order to receive the super early bird registration rate. Registrations received between 23 January 2012 and 27 April 2012 must be paid in full by 27 April 2012 in order to receive the early bird registration rate. Registrations received between 27 April 2012 and 23 October 2012 must be paid in full by 23 October 2012 in order to receive the regular registration rate.
Failure to adhere to the deadlines will result in your registration being automatically moved to the next registration rate.
Cancellation of Registration Deadline: 29 September 2012
Cancellations must be made in writing to the Congress secretariat. Cancellations received prior to 29 September 2012 will receive a full refund minus a AU$150 administration fee. Cancellations received after 29 September 2012 will not be refunded. The Committee will consider cancellations of registration under exceptional circumstances. Refunds will not be granted on failure of visa application. Substitute delegates will be accepted.

The Congress and its agents reserve the right to alter without prior notice, any of the arrangements, timetables, plans or other items relating to the Congress, for any cause beyond its reasonable control. The iADH2012 Congress Committee and the Congress Organisers are not liable for any loss or inconvenience caused as a result of such alteration. In the event of unforeseen circumstances, the iADH2012 Congress Committee and Congress Organisers do not accept responsibility for loss of monies caused by delays.
Participants are advised to take out personal travel insurance and to extend their policy to cover personal possessions. The Congress does not cover individuals against cancellations of bookings or theft or damage to belongings.

The Welcome Reception is included with full delegate and accompanying person registration. Additional tickets are available for purchase for other guests.
The Welcome Reception will be held in the Congress Exhibition and will present an opportunity for delegates to meet outside of the formality of Congress sessions.

Congress Gala Dinner, Tuesday 30 October 2012

The Congress Gala Dinner is included with full delegate and accompanying person registration. Additional tickets are available for purchase for other guests.
The Congress dinner will be a highlight of the social program. During the dinner, delegates will be treated to some local talent and get a real flavour of Australia. The experience will be further enhanced by superb food and wine showcasing some of the regions finest produce. A feast for the senses, there will be something for everyone at this incredible event.
